How to Access Care at the Health Clinic

If you are experiencing symptoms of  please let the front desk know as soon as you arrive.  

For most illness- and injury-related visits, you don’t need to call ahead—just come by the Health Clinic during our walk-in hours. 

All patients should bring the following to their first appointment: 

  • ètv ID card
  • Health insurance card
  • Cash, check or credit card to pay any co-pays*

*  If you are a student who needs care and you can’t pay your co-pay at the time of service, let us know and we can charge your student account for the co-pay.  

If you have scheduled an appointment, please arrive 15 minutes before your scheduled appointment time to allow for the completion of the necessary paperwork.  

If you have past records or prescriptions that you feel will be useful for your provider in planning your care, please bring them with you. If you are unable to make your appointment time, please call as soon as possible to reschedule. 

  • Will  you be under the age of 18 at the time of service?

    If you are under 18 years old, there are certain services you may consent to without parent or guardian involvement.  These include:  

    • Birth control information and contraceptives
    • Pregnancy testing, prenatal care, and pregnancy services
    • Testing and treatment for sexually transmitted infections
    • Substance abuse treatment
    • Outpatient mental health counseling/psychotherapy
    • Inpatient mental health treatment   

    Please talk to your healthcare provider if you have questions about what information will be confidential.  Minors seeking care for other conditions must work with a parent or guardian to submit a Consent to Treat a Minor Student Form. If getting this form completed poses a challenge for you, please reach out to us for guidance prior to your visit. 

  • Need parking?

    If you have a ètv parking permit, street parking is available: 

    • On 10th Avenue or 19th Street
    • In student lots to the north and south of Cassidy Hall 

    If you don’t have a ètv permit, there are a limited number of parking spaces located behind Cassidy Hall that are marked "Health Clinic Patient Parking.”  In order to use these spaces, you must request and display a permit from the Health Clinic front desk. Be sure to make this request as soon as you arrive. Vehicles without a permit will be ticketed and towed.  

    There are designated accessible parking spaces on the northside of Cassidy Hall on 19th St and on the west side of Cassidy behind the Health Clinic. If you are unable to walk into the Health Clinic, call the front desk at 970-351-2412 for wheelchair assistance.
